Questionnaire For Consent Order Fast Track

This questionnaire will take about 20 minutes to complete. Please allow yourself enough time to finish it before starting.

Before you begin, have all your financial details to hand. We will use the information you provide to fully advise you and complete your application to the court for a consent order.

We aim to have your documents ready for signing seven days after receiving your fully completed questionnaire.

We want to provide you with the very best service, so please help us do this by making sure that you answer all of the information fully. Submitting a questionnaire with incomplete or missing information will delay the process.

You have a duty to the court to give a full, frank and clear disclosure of all your financial and other relevant circumstances. A failure to provide full and accurate disclosure may result in any order the court makes being set aside.

If you are found to have been deliberately untruthful, criminal proceedings may be brought against you for fraud under the Fraud Act 2006.

Before you begin

We want to get your consent order completed and approved by the court as quickly as possible. The court will only approve your consent order if they have the information from the following questions. Please help us prepare your consent order as quickly as possible by answering the following questions for you and your spouse. This is a fully transparent process, so both parties have visibility of each other declared assets before signing the documents for the court's approval. Even though it's a fully transparent process, we understand that some couples want to complete their own questionnaire.
